Outreach Manager

This role will be to develop and create working relationships with communities. Meeting new case managers, potential clients, attending important meetings to sell the services of the company. The OM will work with both the DM and CLM to create new services within the company. The OM will maintain new developments for the first 45 days of client caseloads and care before passing it along to the CLM. The OM will be responsible for uploading everything into the HHAExchange system.

    •    Ensure growth in the area of 24 hour supervision supports increases annually and reflects 33% of all new business
    •    Develop connections with counties, hospitals and teams to increase referral sources for people who need higher levels of care
    •    Bring company to maturity utilizing current and new assets
    •    Look for unique opportunities for expansion of the company through acquisition, or development of new services
    •    Ability to track a development from Assessment to Intake and ensure all proper licensing requirements have been met.

Required Skills and Experience (Minimum requirements in terms of educational background, work  experience, licenses/certifications or other knowledge skills and abilities) 

(1) a bachelor's degree in a field related to human services, and one year of full-time work  experience providing direct care services to persons with disabilities or persons age 65 and older; (2) an associate degree in a field related to human services, and two years of full-time work  experience providing direct care services to persons with disabilities or persons age 65 and older; (3) a diploma in a field related to human services from an accredited postsecondary institution  and three years of full-time work experience providing direct care services to persons with  disabilities or persons age 65 and older; or 

(4) a minimum of 50 hours of education and training related to human services and disabilities;  and 

(5) four years of full-time work experience providing direct care services to persons with  disabilities or persons age 65 and older under the supervision of a staff person who meets the  qualifications identified in clauses (1) to (3)

(6) a minimum of three years of supervisory level experience in a program providing direct support  services to persons with disabilities or persons age 65 and older 

Additional Skills and Experience (Preferred or helpful)  

Experience in 245D licensing requirements preferred (Required)

 • Minimum of 2 years degree in Social Service/Human Service Field (Required)

 Minimum of 1 year experience working with case managers and clients (Preferred)

Minimum experience of 1 year working as an intake specialist (Required)

• Knowledge of 245D paperwork (creating/developing SPA, IAPP, SMA, etc) (Required)
